TB2. Evaluarea eficacitatii locale a hialuronatului de zinc in tratamentul ulcerelor venoase de gamba

Studiul a inclus 20 pacienti de ambele sexe cu o medie de varsta de 56 ani, suferind de ulcere de gamba datorate varicelor hidrostatice sau sindromului post-trombotic.

Durata tratamentului cu lotiune-emulsie de hialuronat de zinc (preparatul Curiosin® al firmei Richter Gedeon) a fost in medie de 6 saptamani (5-8 saptamani).

Pentru o mai buna urmarire a procesului de epitelizare si vindecare au fost selectati pacienti cu ulcere de gamba fara detritusuri celulare pe suprafata lor. Hialuronatul de zinc a fost aplicat zilnic sub pansament steril. Mentionam ca a fost singurul topic folosit. Preparatul a fost foarte bine tolerat, neinducand reactii adverse sistemice, alergice sau ortoergice. Stimularea procesului de epitelizare vindecare a survenit la cele mai multe cazuri studiat, fiind evidenta in saptamana 2-3 de tratament. Procesul regenerativ a fost stimulat concentric sau sub forma de "promotorii" epiteliale marginale. Important a fost si modul de aplicare al preparatului dupa o buna curatare a ulcerului si indepartarea exudatiilor zemuinde sau uscate.

Procentajul rezultatelor bune si excelente poate fi estimat la 75% din cazurile studiate. Evolutii satisfacatoare au fost inregistrate la 20% din cazuri dar in restul de 5% rezultatele au fost neconcludente sau pacientii au fost eliminati din studiu din motive obiective.

Hialuronatul de zinc poate fi inclus in terapia ulcerelor de gamba, fiind un agent topic eficient, neiritant, usor de aplicat, stabil si economic pentru utilizare zilnica, fiind util chiar si in cazurile de ulcere cu evolutie indelungata, cu margini caloase, hipertrofice, rezistente la alte metode de terapeutice.

Evident aceste rezultate trebuie sa fie confirmate de studii viitoare realizate pe un numar mai mare de pacienti.

TB2. Evaluation of the local efficacy of Zinc Hyaluronate in the treatment of venous ulcers of the calf

The study included 20 patients of both sexes with a mean age of 56 years, affected by venous ulcers of the calf due to varicose veins or post-thrombotic syndrome.

The duration of the treatment with Zinc Hyaluronate lotion-emulsion (Richter Gedeon's Curiosin® formulation) averaged 6 weeks, ranging from 5 days to 8 weeks.

Ulcers with a clear base and without tissue debris were selected for a better follow-up of the epithelization-healing process.

Zinc Hyaluronate was applied on a daily basis, under a sterile dressing, and was the only topical agent used.

The formulation was very well tolerated, leading to no local or systemic adverse reactions, allergic or orthoergic.

Stimulation of the epithelization-healing process occured in most of the studied cases, starting roughly with week 2 or 3 of treatment. The regenerative process was stimulated either concentrically, or as marginal epithelial "promontories".

The application of the formulation after a thourough cleansing of the ulcer, with removal of any weeping or dry exudations, seems important.

The percentage of good and excellent results can be estimated at 75% of the studied cases; satisfactory evolutions were recorded in about 20% of the cases, the rest of 5% of the cases being inconclusive or study dropouts for objective reasons.

Zinc hyaluronate may be included in the management of venous leg ulcers, being an efficient, non-irritating, easy to handle, stable, economical agent for topical daily application, even on long-standing ulcers with callous borders, resistant to other therapeutic methods.

Obviously, these results need to be confirmed by further studies on larger numbers of patients.
