TA3. Terapia puls in onicomicoza: avantaje, limite, contraindicatii
Onicomicoza reprezinta 50% din toate afectiunile unghiale si 50% din toate afectiunile cutanate si ale fanerelor. Onicomicoza poate fi cauzata de dermatofiti, non-dermatofititi si levuri (candida).
Esentialul pentru diagnosticul de onicomicoza este examenul nativ micologic si insamantarea pe medii de cultura: Sabouround, mediu Mycosel, DTM.
Pacientii care au onicomicoza prefera terapia orala fata de tratamentul local pentru ca este mai simplu si mai eficient.
Datorita proprietatilor farmacocinetice itraconazolul poate fi folosit in terapia puls a onicomicozei (2 x 200 mg/zi, 7 zile pe luna, 2 pulsuri in onicomicoze la mana; si 3 pulsuri in cele de la picior).
Terapia puls cu intraconazol ofera avantaje asupra compliantei pacientului, asupra profilului de siguranta la itraconazol si asupra costului tratamentului.

The onycomycosis represents 50% from all the nail affection and 10% from all the cutaneous and superficial body growth affections, The onychomycosis maybe caused by dermatophyts, nondermatophyts and yeast (candida). The native mycology examination and the insemination on culture environments: Sabouround, Mycosel environments, DTM are essential for the diagnosis of onychomycosis. The patients who suffer from onychomycosis prefer the oral therapy instead of the local treatment because it is more simple and more efficient.
Because of the pharmaco-kinetic the Itraconazol may be used in pulse therapy of the onychomycosis (2 x 200 mg/day; 7 days on month, 2 pulses in onychomycoses athand; and 3 pulses in those from the foot).
The pulse therapy with Itraconazol offers advantages on the patient compliance; on the security profile at Itraconazol and on the treatment cost.
