R3. Tratamentul cu Dapsona in boala Kaposi
Este cunoscuta eficacitatea Dapsonei in boli infectioase (lepra) si in afectiuni inflamatorii cronice. In ultimul deceniu a fost semnalat efectul favorabil al Dapsonei initial in boala Kaposi asociata HIV, ulterior si in formele clasice de boala. Modul de actiune al Dapsonei in angiosarcomatoza Kaposi nu este cunoscut. Ar putea fi implicate protectia fata de procesele distructive tisulare autooxidative si interferenta cu imunitatea T-celulara, alaturi de fectul antimicrobian.
La randul nostru am incercat tratamentul cu Dapsona la pacientii cu boala Kaposi. Prezentam studiul prospectiv deschis realizat pe un lot de 8 bolnavi (7 barbati si 1 femeie, cu varsta medie 60 ani) cu angiosarcomatoza Kaposi forma clasica, tratati cu Dapsona
100 mg/zi. Durata administrarii medicamentului a variat pentru fiecare pacient in functie de momentul includerii in studiu, fiind cuprinsa intre 2 luni si 2 ani. In toata aceasta perioada bolnavii au fost strict monitorizati, in special hematologic. Nu s-au inregistrat efecte adverse.
Evolutia a fost favorabila la toti bolnavii, intre 25-75% din leziuni fiind ameliorate.
Intr-un caz leziunile s-au aplatizat considerabil, chiar pana la disparitie in unele teritorii.
Pacientii sunt inca in urmarire, dar rezultatele incurajatoare obtinute ne fac sa credem ca Dapsona este un mijloc terapeutic eficient in boala Kaposi.
R3. Dapsone therapy in Kaposi's sarcoma
Dapsone is used in leprosy and in several chronic inflammatory dermatoses without infectious aetiology.
In the past decade Dapsone was used succesfully first in cases of HIV - related Kaposi's sarcoma and then in classic forms of disease too. The mode of action of Dapsone in Kaposi's sarcoma is unknown. It could be the fact that Dapsone provides protection from auto-oxidative tissue damage and interfers with T-cell immunity, in addition to its antimicrobial activity.
We tried Dapsone therapy in patients with Kaposi's sarcoma too. We present thus our open prospective study of 8 patients
(7 men, 1 woman, mean age = 60 years) with classic form of Kaposi's sarcoma treated with Dapsone 100 mg daily for at last 2 months
(2 years at the most). The patients were strictly monitorized. No side effects have been noted. All the patients responded well to the treatment with 25-75% improvement of the lesions. In one case the lesions had flattened considerably with totally disappearance in some zones.
The patients' follow-up continues but the results make us to think that Dapsone have a beneficial effect in Kaposi's sarcoma.
