Prezentam un caz de hialinoza cutanata, limitata la fata si gat aparuta la varsta adulta.

Bolnavul, in varsta de 64 de ani, se prezinta pentru deformarea fetei si nasului, aparuta de 2 ani, sub forma accentuarii orizontale a pliurilor frontale, ingrosarea tegumentelor obrajilor si deformarea nasului prin confluarea unor papule, care-i confera aspect muriform, asemanator rinofimei. Pe gat si pe decolteu prezinta o eruptie profuza de papule, de culoarea pielii normale.

Examenul general nu a oferit modificari patologice.

Radiografia craniului a evidentiat calcificari retro- si lateroselare.

Proteina Bence-Jones a fost negativa.

Examenul histopatologic: coloratia cu hematoxilina-eozina a evidentiat mase omogene subepidermice slab colorate cu eozina. Coloratia cu albastru de toluidina a aratat slaba metacromazie. Van Gieson arata slaba coloratie cu fuxina acida. Fibrele elastice lipsesc. Reactia PAS - pozitiva.

Vom prezenta examenul electronomicroscopic.

Cazul pare a fi o hialinoza cutanata neereditara, cu aparitie la varsta adulta.

We are presenting a case of cutaneous hyalinosis limited at the face and neck, occurred in adulthood.

The patient, 64 years old male, presents for the deformation of the face and nose, suddenly developed within the last 2 years, as a horizontal enlargement of frontal folds, thickening of the cheeks and nose deformation by the confluence of the papules in a blackberry appearance rinophima-like. On the neck and low-neck we see a profuse rush of skin-colored papules.

The general investigations didn't show pathological alterations.

Skull X-ray showed retro- and lateral calcifications of the sella turcica.

The Bence-Jones protein was absent.

The routine lab-investigations normal values.

Histologically the H&E stains highlighted subepidermal homogeneous masses, poor colored with eosine. The blue-toluidine stain showed poor metachromasia. The Van Gieson stain shows poor coloration with acid-fluxine; the lack of elastic fibers. The PAS-reaction is highly positive. The Alcian-blue method is poor positive. We shall present the M. E. investigation.

This case seems to be a cutaneous hyalinosis nonhereditary with sudden appearance in adulthood.
