PE8. Oxigenarea hiperbara - aplicabilitate in dermatologie

Oxigenoterpia hiperbara (OHB) este o metoda de tratament in care bolnavul, introdus intr-un cheson, inhaleaza oxigen la presiuni mai mari decat presiunea atmosferica (760 mmHg sau 1 atm. abs).

Scopul OHB este de a furniza oxigen tesuturilor organismului (in contact cu sangele bogat in oxigen), aflate intr-o stare de hipoxie. In conditii normale tesuturile extrag 6 ml O2 din 100 ml de sange (diferenta arterio-venoasa), extragerea realizandu-se initial pe sema O2 dizolvat in plasma si apoi pe cea a O2 legat de hemoglobina.

In OHB la presiuni de 2,8 atm abs., oxigenul arterial are valoarea de 26 ml O2/100 ml sange, moment in care tesuturile preiau numai oxigenul dizolvat in plasma, fara sa utilizeze O2 combinat cu hemoglobina, care ramanand saturat, produce arterializarea sangelui venos. De asemenea efectele toxice ale oxigenului sunt foloite in scop terapeutic pentru distrugerea unor germeni (ex. Clostridii) sau tesuturi.

Lucrarea isi propune sa evidentieze afectiunile in care OHB si-a dovedit eficacitatea, precum si modul in care OHB si-a dovedit eficacitatea, precum si midul in care ea actioneaza asupra pielii. Dintre indicatiile de aplicare ale OHB mentionez: garantarea gazoasa, infectii cutanate mixte cu germeni aerobi si anaerobi, ulcere ale membrelor inferioare fara tendinta la cicatrizare de diferite etiologii: arteriale, diabetice; plagi radionecrotice, actinomicoza, arsuri, degeraturi, plastiile cutanate.

De asemenea, lucrarea demonstreaza ca OHB reprezinta o alternativa in tratamentul unor boli refractare la terapia conventionala.

Hyperbaric Oxygen Therpy (HBO) is a treatment method in which the patient, introduced in a hyperbaric chamber, inhale oxygen at pressures higher than atmospheric pressure (760 mm Hg or 1 ata). The aim of HBO is to supply oxygen in body's tissues (in contact with rich in oxygen blood), being in a hypoxic state. In normal conditions tissues extract 6 ml O2 from 100 ml blood (arterio-venous difference), the extraction initially coming true plasma dissoved oxygen, and after through oxygen bound to haemoglobin. In HBO, under 2.8 ata pressures, the arterial oxygen is 26 ml/100 ml blood; in this moment the tissues take over only the oxigen dissolved in plasma, without using oxygen bound to haemoglobin, which remaining saturated lead to arterialization of venous blood. The oxygen's toxic effects are used for destruction of some germes (i.e. Clostridium) or tissues.

The paper proposes to spotlight diseases in which HBO proved its efficacity, and also its mechanism of action at skin level. The main indications of HBO in dermatology are: gas gangrene, mixed skins' aerobic and anaerobic infections, refractory ischaemic problem wounds, diabetic wounds, radiation necrosis, actinomycosis thermal burns, skin grafts. The paper proves that HBO is an alternative in some diseases which are refractory to conventional therapy.
