Se prezinta cazul unei paciente de 73 de ani tratata de aproximativ 3 ani cu diagnosticul de kraurozis vulvar, fara rezultat. Bolnava prezinta: prurit intens persistent neinfluentat de terapie si modificari ale hemivulvei stangi realizand o placa eritematoasa si pigmentata net delimitata, neinfiltrata, nespecifica. Examenul ginecologic repetat a fost in limite normale. La fel, examenul clinic subiectiv si obiectiv al rectului si aparatului urinar. Examenul histopatiologic a aratat prezenta de celule Paget la nivelul epidermului, permitand stabilirea diagnosticului de boala Paget extramamara primara-vulvara. S-au efectuat coloratia PAS si coloratia cu albastru de toluidina care au fost pozitive.

Bolnava a fost tratata in doua etape: s-a facut electrodesicatia placii si dupa epitelizare, Rx-terapie. La 4 luni dupa terminarea terapiei starea pacientei este foarte buna: nu sunt semne de recidiva si nu prezinta prurit.

We present the case of a 73 years old women who has been unscuccessfully treated for about 3 years with the diagnosis of kraurozis vulvae. The patient presented an intense and persistent itching unresponsive to therapy and discrete changes of the skin of the half left portion of the vulvae forming an erythematous pigmented nonspecific patch with sharp irregular border. Repeated gynecological investigations proved normal. Similarly, no rectal and urinary bladder lesions were found. Histopathological slides showed Paget's cells within the epidermis. Stains with PAS and toluidin-blue showed positive. The diagnosis of primary extramamary-vulvar Paget disease was established. the treatment involved electrodistruction of the patch followed by RXtherapy. Four months later no itching and no recurrences were noted.
