PC2. Lupusul eritematos si sarcina
Incidenta crescuta a lupusului eritematos la femeile tinere si posibilitatea existentei unei sarcini la aceasta varsta ridica probleme speciale de incadrare, terapie, dispensarizare si sfat genetic. Cu ani in urma textele medicale erau unanime in a contraindica sarcina la pacientele cu lupus. Diagnosticul nuantat corelat cu investigatiile imunologice de finete permite azi o atitudine mai flexibila in aceste cazuri.
Prezentam doua paciente cunoscute cu lupus eritematos cronic, gravide in primul trimestru de sarcina, la care se constata o accentuare a leziunilor cutanate. S-a pus problema mentinerii sau nu a sarcinii. Intr-un caz s-a constatat lipsa anticorpilor RO si prin urmare s-a indicat mentinerea sarcinii, iar in cel de-al doilea caz prezenta anticorpilor anti RO a impus indicatia de intrerupere a sarcinii. Autorii trec in revista cele mai noi date din literatura medicala privind aceste asocieri.
PC2. Lupus Erythematosus and the Pregnancy
The increasing incidence of Lupus erzthematosus (LE) among young women and the possibility of a pregnancy at this ageraises special problems of diagnose, therapy, hospitalization and genetic counseling. Years ago the medical texts were unaniomus in discouraging pregnancy for patients with lupus. Today the nuance diagnostic coordinated with finetuned immunologic investigations allows a more flexible attitude in those cases.
We describe two patients in the first three months of pregnancy who suffer from lupus erythematosus with increased skin lesions. In one case the absence of anti RO antibodies was apparent, thus the decision was to go on with the pregnancy, and in the second case the anti RO antibodies were present, so the choice was to terminate the pregnancy. The authors review the newest data avaiable from the medical literature concerning those associations.
